MATT'S OUTBACK PAINTBALL LLC. WAIVER SUMMARY *NOT OFFICIAL USE*

In consideration of being allowed to participate in any way in paintball activities and related events and activities, the undersigned, for themselves, their personal representatives, heirs and next of kin:

1. Acknowledge, agree, and represent that they understand the risks associated with participating in paintball activities and that these risks may result in physical or emotional injury, paralysis, death, or damage to themselves, to property, or to third parties. They further acknowledge that these risks may be caused by their own actions, or inactions, the actions or inactions of others participating in the event, the rules of the event, or by the conditions in which the event takes place.

2. Acknowledge, agree, and represent that they understand that paintball activities may involve a test of a person's physical and mental limits and carry with it the potential for death, serious injury, and property loss. The undersigned further acknowledge that they are qualified, in good health, and in proper physical condition to participate in such activity.

3. Acknowledge, agree, and represent that they will comply with the stated and customary rules for participation in the event and will adhere to the instructions or directions of Matt's Outback Paintball, LLC. and its employees, representatives, and/or agents.

4. Acknowledge, agree, and represent that prior to participating in the event they will inspect the facilities, equipment, and paintballs to be used, and if they believe anything to be unsafe, they will immediately advise Matt's Outback Paintball, LLC. of such conditions and not participate.

5. Acknowledge, agree, and represent that they are fully aware of the risks associated with participating in the event and are willing to assume those risks and accept sole responsibility for any and all damages, liabilities, losses, or expenses that they incur as a result of participating in the event.

6. Release, discharge, and covenant not to sue Matt's Outback Paintball, LLC. or their respective administrators, directors, agents, officers, members, volunteers, and employees from any and all claims, demands, or causes of action that they may now or hereafter have for injuries, damages, or loss to person or property arising out of or relating to their participation in the event.

7. Agree that this Waiver To Play shall be binding on their heirs, next of kin, executors, administrators, and representatives in the event of their death or incapacity.

8. Agree that any pictures or videos taken during the event may be used for promotional purposes.

9. Agree that the terms of this Waiver To Play shall serve as a release and assumption of risk for their minor child/children.

NOW, THEREFORE, in consideration of being allowed to participate in the event, the undersigned hereby acknowledge that they have read and voluntarily agreed to the terms of this Waiver To Play and Release of Liability.
